Instructions:

1. Purchase eMemorial. Purchase price is for first 30 days, after that eMemorial is for perpetuity at no charge.

2. Book Mark this Page. Gather information together, then submit.

3. Submit Your Material

Submission in TWO PARTS:

3A. PART ONE fill out Form and click submit.

3B. PART TWO attach Photo and Files to an email and send.

Other Ways to Submit:

3c. This Form (please follow up with photos attached in an email to us or by mail).

3d. By email. Just email your information with files and photos attached to:

3e. Postal Mail. You may use regular mail. Send us photos and other material (Items can NOT be returned) to:

4. Before Proceeding:

4a. You must have your own permanent email address. If not, you may open a free email account with a provider such as yahoo.

4b. Know the correct spelling of the Deceased's First and Last Name.

Once submitted, your email address and the deceased's name can not be changed.

5. Any photo or music submitted or sent, is a statement by you that you have the RIGHT TO USE said material. That you own the copyright or a licence to use it. Purchasing a music CD or downloading a song, does not give you the right. You must own the copyright or have a royalty free licence or proof of public domain. Photographers own the copyright to every photo they take, even if you are the subject, you need to obtain their permission. We do not dispense legal advise and will not determine what you may or may not use. It is your responsibility to obtain and keep proof of your right to use said material.

6. After submission we will custom create an eMemorial for you. Usually within 3 to 10 days. We will notify you by email when it is finished. You may then, within 30 days, request corrections, one time only.

7. The deceased's eMemorial web address will look like this:

ePlot No. (if ePlot box is left blank we will customize for you). Sales box is for office use, leave blank.

Family ePlot: Submit a new form for each person. Some features such as guestbook, music, Life Story, PayPal button etc, must be shared.

Pre-Purchase:

If purchased in advance, write "alive" or "living" in cause of death box. Also write 11/11/1111 for date died.

Insignia:

Fraternal Insignia if person belonged to a society, order, etc. or you can send us any desired small graphic, even an animated .gif file.

File Format:

Photo must be .jpg

Graphic must be .gif or .jpg

Music must be .mp3

5 MB max per file

10 MB max TOTAL
